A child walked out of the facility and went to a neighboring business.
Employees will no longer use doors location on the east side of the building facing 4th Avenue. The back gate has a self closing latch which was not working and will be replaced. An alarm on the upper level door will be installed and the alarm on the lower lever door will be re-coded. Staff members will be spoken to regarding the importance of proper supervision.
Facility needed several repairs.
Facility had several damaged sleeping cots. Some walls in facility had chipped paint and damaged drywall. Duct tape was being used to repair damaged walls.
Damaged sleeping cots were replaced. Chipped paint and damaged dry wall was fixed. Duct tape and glue was removed and replaced with something more permanent.
Provider was not adequately supervising.
Provider was not providing adequate supervision, and several children were bitten by another child. Provider was placing children in a high chair for discipline.
A Child Care Health Consultant visited with the provider and gave education on biting and disciplining.
4 children were bitten within a 2 week period.
Four children were bitten in facility within a 2 week time period. Inspector observed that this likely happened as the result of inadequate supervision of children.
Provider is training another person to ensure adequate supervision of children.
