What is a Health and Safety Inspection?
Most child care providers offer safe and healthy environments. Health and safety inspections and investigations are a normal part of child care in Idaho. Inspections are not intended to "catch" anyone doing something wrong. Inspections help ensure the safety of children and staff, support staff in following requirements by finding concerns, and promote continuous improvement.

Incident Reports
What is a Substantiated Incident?
Occasionally, families and community members may report that a child care facility is not meeting requirements for safe and healthy child care. All child care incidents reported in Idaho are investigated. After an investigation, if it is determined by the relevant authorities that an incident occurred, it is considered “substantiated.” It is important to know that only substantiated incidents and allegations are published on this website. Incidents that are still under investigation will not be published until the investigation is concluded.
Idaho began publishing substantiated incidents in October 2017. Incidents are published as each investigation is concluded and maintained on this site for at least three years.
Safe Sleep Concerns, Supervision Concerns
There were concerns that this provider is frequently out of ratio and that they do not follow safe sleep practices in the infant room. Upon investigation, it was determined that the facility within child/staff ratios. However, during the investigation, infants were sleeping with loose blankets and a pacifier clip which is out of compliance with safe sleep guidelines.
Resolution
The health department informed the provider that these hazards needed to be removed immediately and staff needed to be trained on safe sleep practices. Safe Sleep handouts were provided.
Discipline & Behavior Concerns
There were concerns that the owner of this facility often places children in "chokeholds" or puts them in utility closets as a form of discipline. During an investigation with law enforcement, this provider admitted to physically restraining children as a means of behavior management but denies the use of any "chokehold". There was no evidence of any crime that occurred but serious concerns about the safety in the facility for children was noted.
Resolution
Based on the inappropriate forms of behavior management and safety concerns, the Department has issued a Notice of Concern to this provider.
Supervision Concern
There were concerns regarding a child leaving the outdoor play area which leads to a busy road. During the investigation, it was determined that a child had climbed over a wall and attempted to leave the premises.
Resolution
The provider extended the wall in the outdoor play area another foot at the top to prevent this situation from occurring again.
